SMS modules

Flight risk, hazards and equipment, in the same evidence chain.

Three optional modules extend line observation into a full safety program. Turn each one on in department setup, customize its form like a checklist, and map every item to IS-BAO, Part 5 SMS or your own FOM.

Flight risk assessment (FRAT)

  • Built in the same editor as your checklists: risk factors, answer choices, points and score thresholds, each mapped to standards.
  • Step 1 before the flight: the crew answers each factor, sees the score and level live, and writes a mitigation plan when the level requires it.
  • High scores go to the roles you choose for release before the flight.
  • Step 2 after the flight: the crew selects the threats, errors and undesired states they met and how each was handled.
  • Analysis by risk level, month, most frequent factors and post-flight threats and errors, with CSV export.

Hazard reporting

  • A separate entry for every member, with categories, questions and a 5 by 5 risk matrix you set to your SMS manual.
  • Reference numbers, committee assessment, mitigation status, links to committee actions and a response to the reporter.
  • Anonymous reporters get a tracking code to read the committee response.
  • Analysis by category, risk matrix, status and time to close, with CSV export.

Equipment inspections

  • Any equipment: aircraft documents and emergency equipment, inventory, AEDs, fire extinguishers, calibrated tools, vehicles or facilities.
  • Custom intervals and grace periods, assigned to members with the Equipment inspector role.
  • Every failed item becomes a discrepancy with severity, an airworthiness flag, deferral with reason and a fix record. Airworthiness items need a maintenance record or work order reference to close.
  • Compliance tracking: on-time rate, overdue schedules, open discrepancies and time to fix.

Anonymous or identified: your choice

For FRATs and hazard reports, department setup lets you choose whether records are anonymous, like observation data, or traceable to the crew member. Traceable records let the committee send positive recognition for good risk management and good reports. Anonymous FRATs keep a temporary link only until the crew finishes the post-flight step, then the link is deleted.

One audit, every module

The standards report counts FRAT, hazard and inspection records against the requirements their items are mapped to. Run it for one standard at a time to see coverage and gaps for IS-BAO, then for Part 5 SMS, then for your company FOM.

Standards Captain organizes evidence. It does not certify compliance or replace a maintenance tracking program.
